  • Enhanced DBS check required
  • Speak English to an appropriate standard
  • Experience in health and social care settings
  • Skills in work prioritisation, risk identification, workload organisation, and knowledge of statutory frameworks, social care legislation, council policies and procedures
  • Ability to work autonomously and as part of a team
  • Prepared to commence work with required legal documents (valid visa for eligible applicants); note that the employer does not sponsor visas

Responsibilities

  • Assist in the completion of service user and carer assessments of care needs and undertake reviews
  • Coordinate care planning, commissioning and provisioning of services to meet outcomes in line with early intervention and strengths‑based social work
  • Manage an individual caseload while working within the Adult Social Care Locality Team to meet service demands
  • Maintain statutory and council procedures, including risk and safeguard reporting
