The predicted salary is between 30000 - 42000 Β£ per year.
To work with children, young people and their families, on a one to one or group basis, using a variety of interventions, to identify needs and to achieve positive outcomes.
To work in partnership with parents / carers to empower them and to enable them to achieve better outcomes for their children.
To work co-operatively with children's services, health agencies and other community based services, to provide the most effective service for children, young people and families.
Undertake developmental and health reviews of children and young people at key stages.
We are seeking a passionate, dynamic and experienced 0-9 yrs practitioner to join our Healthy Family Team. Our teams are fully integrated consisting of Health Visitors, School Nurses, Support Practitioners, Family Hub Practitioners and Community Connectors.
Barnardos has been providing services to some of the most vulnerable children across the UK for over 150 years. We work in partnership with children's social care and health partners enabling the delivery of integrated health and social care services in a number of areas.
We recognise that to really transform children's life chances, they need sustained interventions that meet the diversity of their needs and that participation and co-production must be a key component of any service.
