Healthy Family Support Practitioner - Part time Vacancy (22.5hrs) Working with children and or Young People 0-5 years old and their families in Thundersley and The Castle Point Area. We are seeking a passionate, dynamic and experienced 0-5 yrs practitioner to join our Healthy Family Team. Our teams are fully integrated consisting of: Health Visitors, School Nurses, Family Wellbeing Nurses, Children and Young Persons Practitioners, Healthy Family Support Practitioners, Family Hub Practitioners and Community Connectors. 0-5yrs Healthy Family Support Practitioner Role To work with children, young people and their families, on a one to one or group basis, using a variety of interventions, to identify needs and to achieve positive outcomes. To work in partnership with parents/carers to empower them and to enable them to achieve better outcomes for their children. To work co-operatively with children's services, health agencies and other community based services, to provide the most effective service for children young people and families. Undertake developmental and health reviews of children and young people at key stages of their life Working in an integrated team to support the delivery of the Healthy Child Programme Supporting The Healthy Child Programme Plus
