We are looking for a Care Navigator to provide high quality, patient focused administrative and reception support, ensuring efficient running of the practice. Care Navigators will manage patient contact, assist with clinical and admin workflows, and share responsibilities fairly across the team under the skills mix model.Main duties of the job Patient Contact and CommunicationAnswer and triage phone calls, emails, and online requests.Direct patients to the most appropriate service or clinician.Provide clear, professional information to patients on services and appointments.AdministrationProcess registrations, leavers, and patient data changes.Handle document workflow, coding, and summarising of patient records.Manage referrals and track outcomes.Support chronic disease call/recall programmes.Assist with repeat prescribing processes, following protocol.Produce insurance reports, SAR requests, and other correspondence.Reception DutiesGreet and check in patients, manage appointment bookings.Support the smooth running of daily clinics.Maintain confidentiality and GDPR compliance.Team and Practice SupportParticipate in rota for late finishes and cover for colleagues.Work flexibly to support other areas of the practiceAbout us Barlow Medical Centre is a large practice with a very friendly team, We have 5 Partners, Management Team, Salaried GPs, Nursing Team, Prescribing Team, Data Team, Secretarial Team, Care Coordinators, and a Reception Team.
