Care coordinators provide extra time, capacity and expertise to ensure that appropriate support is made available to patients and that their needs are addressed. This is an exciting opportunity for the right person who wants to work supporting people by coordinating their health and social care services. This vacancy is for 15 hours per week [Monday to Friday; 08:00 - 11:00].

Main duties of the job include:

  • Supporting the clinical team to proactively identify and work with patients, including the frail/elderly and those with long‑term conditions.
  • Providing proactive, person‑centred care planning.
  • Helping coordinate care by bringing together the different specialists whose help that individual might need.
  • Involvement with a wide range of services, such as hospital care, community care, social care, housing and the voluntary sector.

About us: Rock Healthcare is a long-established, forward‑thinking and dynamic GP Practice currently serving around 9,500 patients. We are based at Moorgate Primary Care Centre in Bury. The site is a modern, purpose‑built premises, containing a variety of primary care services to meet the needs of the local population. We are proud to be a teaching practice for medical students, nursing students, health care assistants and GP trainees. We are committed to the continued development of our teams.

Job responsibilities include:

  • Helping patients and their families understand, manage and utilise personal care and support plans.
  • Contributing to increasing the number of patients with personal health budgets.
  • Working closely with Primary Care Network (PCN) and GP Practice to ensure patients receive coordinated care, which is responsive to their needs and designed in cooperation with patients, carers, and all supporting services.

Person Specification:

Qualifications:
  • Educated to GCSE level or equivalent.
  • Health and social care qualification.
  • Training around care planning and coordination.
  • Training around residential care.
Experience:
  • Experience of working in health, social care and other support roles in direct contact with people, families or carers.
  • Experience of working in a multi‑professional team environment.
  • Good written English and excellent record keeping skills.
  • Proficient in use of EMIS.
  • Confident in using Email and sound IT knowledge.
  • Experience of working as a Care Coordinator.
  • Experience or training in personalised care and support planning.
  • Experience of working with elderly or vulnerable people, complying with best practice and relevant legislation.
  • Experience of working with reports in EMIS.
Knowledge and Skills:
  • Computer literate with a working knowledge of Microsoft Office packages and EMIS.
  • Understanding of and commitment to equality, diversity and inclusion.
  • Knowledge of policies and procedures, including confidentiality; safeguarding, long working, information governance and health and safety.
  • Knowledge of how the wider NHS works, including PCNs.
  • Knowledge of long term conditions and the complexities involved: medical, physical, emotional, and social.
  • Willingness to work flexible hours when required to meet work demands.
