At Barchester Healthcare, we are undergoing a significant period of growth and transformation, including the integration of newly acquired services into our established operating model. We are seeking an experienced Transition Manager to lead the end‑to‑end transition strategy. This is a pivotal role within our transformation function, sitting at the heart of business and technology integration. You will be responsible for ensuring seamless transition across people, process, systems, and operations, delivering Day 1 readiness and long‑term operational stability.

Working closely with senior stakeholders across HR, IT, Operations, Finance and Legal, you will bridge the gap between programme delivery and real‑world operational impact, ensuring that change is implemented effectively, sustainably, and with minimal disruption. This is a remote role with UK‑wide travel required.

What You'll Be Doing

  • Transition Strategy & Planning
    • Define and own the overarching transition strategy and multi‑phase delivery plan
    • Establish a robust and credible roadmap covering pre‑ and post‑transfer activities
    • Lead the development of a structured Day 1 readiness framework
  • Governance & Decision Making
    • Establish prioritisation frameworks based on risk, compliance and operational impact
    • Surface risks, trade‑offs and dependencies, providing clear recommendations to senior stakeholders
    • Manage risks and issues, escalating critical items where required
    • Support governance forums including Programme SteerCo
  • Business & Technology Integration
    • Align business operations with system and technology changes
    • Manage complex dependencies between technical delivery and business readiness
    • Ensure integration plans reflect both operational and technical requirements
    • Contribute to programme planning and delivery controls
  • Stakeholder Engagement
    • Communicate complex updates, risks and plans at Executive and Director level
    • Build strong relationships across a matrix organisation
    • Influence and challenge senior stakeholders constructively

About You

  • Proven experience as a Transition Manager or Lead within M&A or large‑scale transformation programmes
  • Strong experience in Day 1 readiness planning and post‑merger integration (PMI)
  • Experience managing both business and technology change simultaneously
  • Demonstrated ability to work at pace in complex, ambiguous environments
  • Professional certifications such as PRINCE2, MSP, Prosci or equivalent
  • Exceptional structured thinking and problem‑solving capability
  • Strong commercial awareness with the ability to balance risk, cost and delivery pace
  • Excellent stakeholder management and influencing skills at senior level
  • Comfortable operating both strategically and in the detail when required
  • Resilient, adaptable and delivery‑focused mindset
  • Willingness to travel across the UK with a full driving licence
