As Wellness Coordinator at BaptistCare's Gracewood Village in Kellyville, you will be the person residents turn to when they need support, guidance, or simply someone in their corner. You will assess their needs, connect them with the right services, and build genuine relationships that make a real difference to how they experience life in the village. This is a Monday to Friday, full-time role in a close knit village community.
You will work alongside our Care Facilitator on site, which means you'll have a strong support network around you and a genuine team dynamic from day one.
About the Role:You will be the primary wellness and support contact for residents across Gracewood Village, working closely with the on site Care Facilitator. Your focus is on understanding each resident as an individual and making sure they have access to the services and support that help them live well and independently.
- Assess the individual support needs, goals, and wellbeing of residents across the village
- Develop and maintain support coordination plans and connect residents with appropriate services
- Raise awareness of available services including at-home support, and facilitate referrals to Home Care where needed
- Support residents to self-manage and maintain positive relationships with carers and family members
- Liaise with government agencies, community organisations, and external service providers on behalf of residents
- Maintain accurate, timely case notes, documentation, and records
- Provide regular updates to the Village Manager on changes to residents' situations
- Foster a village environment that supports resident participation, inclusion, and wellbeing

About YouYou are a genuine people person, self motivated, and someone who thrives in a role where every resident interaction matters. Experience in the at-home or community care sector is preferred, though we welcome applicants from broader aged care, retirement living, or allied health backgrounds.
- Minimum 3 years experience in Retirement Living, Residential Care, Home Care, Community Services
- Experience in the at home care sector is highly regarded
- Demonstrated ability to provide effective, individualised support and achieve positive outcomes for residents
- Strong interpersonal and communication skills, written and verbal
- High level of empathy, patience, and tenacity when working with residents and their families
- Competence in service assessment, care planning, and documentation
- Intermediate to advanced proficiency in MS Office and Outlook
- Current NSW Driver's Licence
- National Police Check (or willingness to obtain)
