Permanent Part-time Role: 4 days per week

About the Role: As a Tailored Support Coordinator, you'll play a meaningful role in supporting tenants and household members to access the services and supports they need to achieve their self-identified goals and sustain their tenancies. Working closely with tenants, community services and sector stakeholders, you'll build trusting relationships, identify strengths and needs, co-design tailored support plans and facilitate access to relevant services through warm referrals and advocacy. You'll also monitor progress, support community engagement and help tenants build wellbeing, social connection and self-management. This is an opportunity to make a genuine difference while working alongside a collaborative Community Services & Housing team to support positive outcomes for tenants and their households.

About you: You're a compassionate, resilient and motivated professional who is passionate about supporting people with diverse and complex needs. You build strong, trusting relationships and take a strengths-based, trauma-informed and person-centred approach. You have strong interpersonal and communication skills, enjoy connecting people with the right supports and are committed to helping tenants achieve their goals and sustain their tenancies.

Essential Qualifications & Experience:

  • Certificate IV in Social Work or Community Services, OR Minimum three (3) years' experience in Social Housing, Community Services or a related discipline.
  • High level interpersonal and administration skills and ability to liaise with key stakeholders.
  • Experience working with high-need and complex clients.
