The Role
The primary responsibility of the Deputy Sister/Charge Nurse is to provide nurse led care to patients being treated in a community setting.
The nurse will work with the patient, their support workers, their families and the multi professional team to develop a care plan that will meet individual needs and promote optimal clinical outcomes.
The nurse will support the patient to manage their health and/or social condition(s) to avoid unnecessary inpatient stay, prevent readmission and unplanned re attendances.
The post holder will ensure that all patients are provided with safe and high-quality standards of nursing care.
They will be aware of the Care Quality Commission standards and support the multi professional team ensuring that they are maintained.
Care, compassion, competence, communication, courage and commitment will underpin all care delivered as well as Standards for Better Health Care, National Service Frameworks, the long-term conditions agenda, hospital avoidance and care closer to home.
The post holder will have responsibility for the assessment of patients referred and to assess suitability for transfer to the service, comprehensive nursing assessments, care delivery, efficient co-ordination ensuring smooth running of service, communication with primary and secondary sector colleagues and discharge planning.
The post holder will initiate and undertake specific projects / audits related to the development of the service, monitoring standards of care, and acting as a change agent for this developing service.
The post holder will initiate and undertake clinical investigations related to the management of the patient within their scope of professional practice.
The post holder will develop patient pathways/protocols in partnership with the multi- professional team to improve the clinical management of patients within the service and into the community.
Main Duties and Responsibilities
- To initiate an agreed range of diagnostic tests within the team’s clinical guidelines/care plan.
- To refer to other disciplines/professions when required.
- To support patients to self-manage, monitor their condition and make informed choices – shared decision making.
- To act as a named nurse for designated patients within the service.
- To ensure that nursing documentation is maintained to required standards and to be a competent user of and participate in the ongoing development of the Trust’s planned Electronic Patient Record.
- To promote the development of evidence-based practice.
- To participate in clinical supervision to an agreed standard.
- To ensure high standards of care are given to all patients.
- To advise patients on the promotion of health and the prevention of illness.
- To be responsible for gaining intravenous access and administration of prescribed medication for patients requiring intravenous medication and for monitoring their care.
- To collaborate with medical colleagues in ensuring that appropriate investigations and procedures are performed to complete the patients’ treatments and that patient and their families are fully informed and understand the purpose for these.
Requirements
- Current NMC Registration
- 6 months of RGN experience at the NHS within the last 3 years
- BSc or equivalent level of experience
- Ability to motivate staff
- Ability to demonstrate awareness of need for effective teamwork
- Ability to demonstrate personal management skills
- Excellent time management
- Computer literate
