Whittington Health NHS Trust are recruiting for a passionate and driven Band 3 HCA to join their highly skilled District Nursing Team based in Islington. Whittington Health NHS Trust provides a wide range of health and care services. From maternity, through children’s mental and physical healthcare, health visitors, community and school nursing and therapists, urgent and emergency, cancer care, dental services, elective care, all the way to end of life care: we support patients, families and local communities in community health centres, GP clinics, dental practices across North Central London and beyond, at our hospital in Archway, and in patients’ own homes.
The post holders will work closely with the district nursing team. You will be expected to provide a high standard of nursing care and contribute to the prevention of unnecessary hospital admissions and facilitate early discharge from hospital. You will work with health professionals providing a high standard of nursing care, which reflects the NVQ3 Care competencies to all patients living in the Community.
Main Duties- Participate in the assessment of care needs and the implementation of programmes of care in partnership with patients/carers in their own homes.
- Identify changes in patients conditions and, within their capabilities, recognise where significant changes in the care plan are required and report these back to the senior nurse on duty in a prompt and timely manner.
- Liaise effectively with other nursing and multi professional team members within the scope of the Health care Assistant role and report back to the nurse in charge.
- Be competent in additional clinical skills and knowledge areas, for example:
- Blood pressure monitoring
- Administration of medicines via gastrostomy plus setting up of gastrostomy feeds
- Administration of oral medication
- Simple sterile dressings
- Best infection control practice
- NVQ Level 3 (Care) or equivalent
- Computer and office skills
- Basic numeracy (drug calculation) skills
- Ability to give appropriate advice to patients and carers
- Ability to use own initiative and at times work unsupervised
- Ability to evaluate and sustain changes for practice
- Ability to function well in times of pressure by prioritising and planning workload
- Flexible approach to task and location and working pattern
- Understanding of Primary Health Care
- Ability to work as part of a team
