As Head Of Service, you will operate at the heart of system integration, reporting directly into a senior transformation director and working across health, social care and the voluntary sector. This is a high‑profile role with real influence, giving you the opportunity to drive prevention, early intervention and neighbourhood‑based delivery at pace.
You will be central to coordinating a new community and neighbourhood model, aligning strategic priorities across multiple partners while navigating a complex and fast‑moving environment. Your role as Head of Health, Care Neighbourhood Coordination:
- Lead coordination of a new Community and Neighbourhood Model across health and social care

- Proven partnership working across statutory, voluntary and community organisations
- Strong system thinking, with the ability to manage complexity and competing priorities
- Track record of delivery, coordinating programmes across multiple stakeholders
- Confidence operating autonomously in high‑profile, politically aware environments
