Step into a role where you will shape how communities experience health and care. As Head Of Service, you will operate at the heart of system integration, reporting directly into a senior transformation director and working across health, social care and the voluntary sector. This is a high‑profile role with real influence, giving you the opportunity to drive prevention, early intervention and neighbourhood‑based delivery at pace.

You will be central to coordinating a new community and neighbourhood model, aligning strategic priorities across multiple partners while navigating a complex and fast‑moving environment. Your role as Head of Health, Care Neighbourhood Coordination:

  • Lead coordination of a new Community and Neighbourhood Model across health and social care
  • Drive prevention and early intervention, translating strategy into tangible delivery

  • Align priorities and funding, including Better Care Fund outcomes and system change
  • Champion partnership working to deliver sustainable, integrated outcomes

Your experience as Head of Health, Care Neighbourhood Coordination:

  • Senior leadership experience within health, social care or system transformation
  • Proven partnership working across statutory, voluntary and community organisations
  • Strong system thinking, with the ability to manage complexity and competing priorities
  • Track record of delivery, coordinating programmes across multiple stakeholders
  • Confidence operating autonomously in high‑profile, politically aware environments
