Balsall Common And Meriden Group Practice is seeking a part-time Chronic Disease Nurse to join our established team. The role focuses on long-term conditions management, with emphasis on diabetes care including GLP-1 and insulin initiation, and respiratory care for asthma/COPD.
The successful candidate will work 27 hours per week, be able to work autonomously, and liaise with the multi-disciplinary team to deliver evidence-based care aligned with QOF targets.
