Our Clinical Homecare Nursing team really love what they do! Working in partnership with our NHS colleagues to provide individualised quality nursing care to patients requiring Parenteral Nutrition and other IV therapies in the comfort of their own homes. This is a unique opportunity for an experienced nurse to work autonomously while delivering high-quality clinical care outside of a hospital setting. Location: Working with trusts and patients in London.
Working hours: Monday to Sunday, 40 hours over 4 days per week on a rolling rota. The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 5 hours in the morning to disconnect patients, and another 5 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week.
Main duties of the job:
- To administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
- To provide training and education to support patients/carers in the connection and disconnection of Parenteral Nutrition and any other prescribed therapies to independence.
- To provide training and education to support patients/carers to use infusion devices to deliver their therapy/therapies independently.
- To support the provision of a phlebotomy/cannulation service for patients.
What you need to succeed:
- Registered Nurse with current NMC registration (adult).
- Current clinical experience minimum of at least 1 year.
- Competent in the management and delivery of IV Therapies.
- Excellent communication skills, time management skills and the ability to work well against strict timescales.
- The ability to make correct clinical decisions in a clinical emergency situation.
- Flexibility to travel and visit patients outside local area.
- Current valid driving licence.

Job responsibilities:
- Provide individualized, compassionate, quality patient care based on empathy, kindness, respect and dignity to all TransCare patients underpinned by current local and national guidelines and policies.
- Demonstrate a proactive approach in being aware of the mental health needs of themselves, their colleagues and patients.
- Demonstrate a flexible approach to support the nurse led service working in accordance with the nursing rota, and the Annual Leave Policy for TransCare Nurses.
- Support the installation of new TransCare patients, complete all relevant documentation and elevate any issues to appropriate internal and external teams.
- Administer prescribed clinical homecare therapies and associated care and support to patients who are unable to be trained to independence, including HPN, IV, MABs, Methotrexate.
- Provide training, education and support to patients/carers in the care and management of their CVAD (Connection, disconnection, prescribed therapies & dressing change) in accordance with local policies and national guidelines to independence.
- Report any clinical changes in patients to the relevant TC nursing team or Regional Lead Nurse or Trust.
- Communicate in an effective and timely manner with appropriate internal and external teams ensuring strict patient confidentiality.
- Take accountability for ensuring all incidents and complaints are raised in accordance with policy, being part of root cause analysis, reflective practice, lessons learnt and future developments.
- Maintain contemporaneous nursing care records at all times in accordance with the NMC's record keeping guidelines, appropriately managing sensitive information.
- Support the TC nursing team and Practice Development Nurses in co-mentoring Bank nurses.
- Act in a professional manner at all times working within the NMC's Code of Professional Conduct.
- Act in the patient's best interest at all times.
- Know own limitations and work within scope of practice.
- Adhere to the relevant BBraun Company policies and TransCare Policies.
- Promote a positive image of the company at all times, showing awareness of professional boundaries.
- Be competent in recognizing and managing a clinical emergency effectively, liaising with all relevant parties appropriately and using clinical judgement to assess and decision make.
- Be accountable for updating own clinical skills, continuous professional development and Revalidation requirements to maintain effective NMC registration.
- Safeguarding Vulnerable Adults and Children take appropriate and timely action in accordance of National and Local policies & procedures.
- To ensure reports from customers relating to product quality complaints or adverse drug reactions are documented and the essential minimum information is collected and forwarded to the Regulatory Affairs Department the same day they are received.
- Adopt the culture of continuous improvement in the TransCare Nursing Service.
- Adhere to the established culture of reflective practice.
- Attend regular performance review meetings with your line manager.
- Any other duties that do not change the nature of this post.
Person Specification:
- RN with current NMC registration (adult)
- Mentoring/Teaching Certificate or equivalent
Experience:
- Current clinical experience minimum of at least 1 year
- Competent in the management and delivery of IV Therapies
- Competent in the care and management of CVADs
- Blood samples from CVADs
- Cannulation/Venipuncture - peripheral
