Working in partnership with our NHS colleagues to provide individualized quality nursing care to patients requiring parenteral nutrition and other IV therapies in their homes. Key Details
- Location: Working with trusts and patients in Plymouth and the surrounding areas.
- Working hours: Monday to Sunday, 40 hours over 5 days per week on a rolling rota. Shifts are split; typical structure 4 hours morning, break, 4 hours evening. Flexibility essential.
- Salary: £42,445 per annum.
Your main responsibilities
- Administer prescribed IV therapies and associated care and support to patients who are unable to acquire independence.
- Provide training, education and support to patients/carers with the equipment and infusion devices required for therapies to independence.
- Support training, education and assistance for connection/disconnection of PN and IV therapies to independence.
- Assist in care and management of their CVAD in accordance with local policies and national guidelines to independence.
- Participate on the on‑call rota, make autonomous decisions to resolve out of hours queries and, if required, escalations to the Senior on‑call.
- Work with Practice Development Nurse and Regional Lead Nurse in discharge management including pre‑discharge assessment visits and home assessments to provide a seamless service.
- Support installation of new TransCare patients – complete relevant documentation and raise issues.
Experience Required
- Intensive Care / High dependency / General surgery nursing experience at a level equal to top of Band 5.
- Current clinical experience minimum of last 2 years.
- Competent in management and delivery of PN and other IV therapies.
- Competent in care and management of CVADs.
- Blood samples from CVADs.
What you need to succeed
- Registered General Nurse with current NMC qualifications.
- Mentoring/Teaching Certificate or equivalent (or attain within 12 months).
- Excellent communication and time management skills with ability to work against strict timescales.
- Ability to make correct clinical decisions in a clinical emergency situation.
- Flexibility to travel to support homecare patients and/or careers is essential.
