Azile Medical are recruiting a Brokerage Assistant for our client in Hackney.

DIRECTORATE: Children, Adults & Community Health

LOCATION: Hackney Service Centre

RESPONSIBLE TO: Senior Brokerage Manager

PURPOSE

Reporting to the Senior Brokerage Manager, the post-holder will have a key role in developing and maintaining databases to support the Brokerage function. This will require close working with a wide range of Care Providers, Voluntary Organisations and Service Users. The Brokerage Assistant will ensure that information about individual placements, support packages and related costs is recorded accurately on information systems and disseminated to the relevant providers and social work/health professionals. The post-holder will be a member of the Commissioning Team and will support Brokerage Officers to source, arrange and amend packages of care, placements in residential and nursing homes and supported living services. This may involve regularly dealing with complex and/or contentious issues. The Brokerage Assistant will follow the policies and procedures set out for the Brokerage Team to ensure that appropriate standards are met and value for money achieved in the procurement and delivery of services. This will include ensuring that all placements are facilitated within the Council’s Standing Orders.

MAIN AREAS OF RESPONSIBILITY

  • To liaise with Social Workers/Care Coordinators, Brokerage Officers, Contract Officers and Providers, Finance and other teams within Adult Social Services to ensure they receive up to date information on contracts and information regarding service users and their care.
  • To monitor compliance of systems by independent care providers and report non-compliance to the Contract Officers.
  • To liaise with providers regarding service provision.
  • Support the development of policies and procedures.
  • To support service users’ and carers’ involvement in all aspects of service delivery.
  • To participate in the Duty system operated by the team filtering and action telephone calls, emails and other communications.
  • To ensure that the Mosaic and CM2000 systems are updated as required.
  • To work effectively with the Information and Assessment Team, Care Management Teams, Hospital Discharge and Finance Teams.
  • To maintain clear lines of responsibility and accountability in carrying out the brokerage function.
  • To maintain accuracy in record keeping and ensure consistent recording practice on Mosaic that will provide robust financial information for the budgets forecasting and community care charging purposes.

  • To undertake any other duties that might arise from time to time as required within the overall grading and general responsibilities of the post.

TECHNICAL EXPERIENCE/ SKILLS/KNOWLEDGE

Service Experience and knowledge:

  • Experience of working within a health or social care context.
  • Experience of residential or supported living placement finding.
  • Experience of managing relationships with stakeholders in a health or social care context.
  • Experience of making effective use of IT systems to record and report on brokerage activity, customer contact and service performance.
  • Able to communicate effectively, confidently and assertively whether in writing or when speaking.
  • Operates effectively as a member of a team.

Personally effective:

  • Ability to make effective use of supervision and contribute constructively to team meetings.
  • Demonstrable ability to carry out work under their own initiative, in an organised fashion, managing own workload.

Technical knowledge:

  • Good working knowledge of the legislation and Council policies covering data protection, information sharing and confidentiality.
  • The Council's 2020 vision, budget strategy and the Government directions, modernising agenda, including Our Health, Our Care, Our Say and the Independent Living Strategy and best value.
  • Commissioning and monitoring techniques.
  • The Community Care Act, the NHS & Community Care Act and related legislation.
  • A working knowledge of the Department of Health guidance in respect of Adult Services.
  • Knowledge of mathematical, graphical and statistical conventions and routines.

Technical skills:

  • Good working knowledge of administrative, contact management and case management systems.
  • Working knowledge of a wide range of typical IT tools including database, word processing, email and spreadsheet applications.

  • Understands political drivers and the role of members.
  • Commitment to continuous learning and development for self and others.

  • Experience of setting targets and delegating tasks.
  • Experience of monitoring performance and suggesting new ways of doing things.

The ideal post holder will have experience of:

  • Working and negotiating with other agencies and other stakeholders.
  • Using financial management and procurement software to commit expenditure.
  • Working as part of a team and able to manage your own workload.
  • Establishing and managing office administrative systems.
