Job type: Full-Time, Fixed Term Contract until September 2027
Working Hours: 37.5 hours per week
Requirements: A full UK driving licence with access to a vehicle and appropriate business use insurance is required.
Duties:
- Build Trusted Relationships: Develop positive, professional relationships that encourage engagement and lasting change.
- Provide Person-Centred Support: Deliver one-to-one support, outreach and practical guidance within community settings.
- Complete Assessments and Support Plans: Produce outcome-focused assessments and co-produced support plans.
- Coordinate Partner Services: Work alongside health, criminal justice, housing and community organisations to ensure individuals receive appropriate support.
- Maintain Accurate Records: Update case management systems and complete reports within required timescales.
- Promote Independence: Encourage confidence, resilience and access to employment, education and community opportunities.
- Manage Risk Safely: Complete dynamic risk assessments and follow lone working and health and safety procedures.
Candidate Requirements:
- Essential: Ability to quickly build rapport while maintaining professional boundaries.
- Excellent written and verbal communication skills.
- Commitment to promoting independence, dignity and informed choice.
- Previous experience maintaining accurate records and performance information.
- Competent using Microsoft Office applications.
- Full UK driving licence with access to a vehicle and appropriate business use insurance.
- Desirable: Previous experience within criminal justice, homelessness, substance misuse or mental health services.
- Personal experience, or experience supporting someone close to you, relating to offending, addiction, homelessness or mental health.
- Ability to motivate and empower individuals to achieve positive outcomes.
