SALARY: £30 to £45 per Hour

LOCATION: Hybrid (Office, Community and Home Working), Devon, South West England

JOB OVERVIEW

We have a fantastic new job opportunity for a Health Practitioner / SEND Trusted Assessor to provide specialist assessment and therapeutic support for children and young people with identified additional needs. This role plays a key part in education, health and care assessment pathways.

As a Health Practitioner / SEND Trusted Assessor, you will deliver trauma-informed and neurodevelopmental assessments, contribute to Education, Health and Care Needs Assessments, and provide therapy aligned with Education, Health and Care Plans. You will work collaboratively within multidisciplinary settings.

This role offers flexible, part-time working with a hybrid model, allowing a balance of home, community and office-based work while making a meaningful impact for children and families.

DUTIES

Your duties as the Health Practitioner / SEND Trusted Assessor include:

  • Clinical Assessments: Completing assessments to inform Education, Health and Care Needs Assessments
  • Therapy Delivery: Providing therapy as outlined within Education, Health and Care Plans
  • Report Writing: Producing clear, transferable clinical reports and recommendations
  • Annual Review Input: Contributing professional advice to Education, Health and Care Plan reviews
  • Outcome-Focused Intervention: Delivering goal-based, time-limited therapeutic work
  • Multi-Agency Working: Collaborating with education, health and social care professionals
  • Care Transitions: Supporting discharge planning and transitions to appropriate services
  • Clinical Governance: Maintaining accurate records and complying with safeguarding and data protection standards
  • Professional Development: Engaging in supervision, reflective practice and ongoing CPD

CANDIDATE REQUIREMENTS

ESSENTIAL

  • Registration with the HCPC in a relevant health profession
  • Previous experience in trauma-informed and neurodevelopmental practice
  • Experience completing assessments and contributing to SEND pathways
  • Proven ability to produce high-quality clinical reports
  • Strong communication, clinical judgement and IT skills
  • Ability to work independently and within multidisciplinary teams

DESIRABLE

  • Experience with attachment-based and trauma-informed models
  • A background in Education, Health and Care assessments
  • Experience supporting children and young people with neurodivergence
