Awburn House Medical Practice is looking to recruit an experienced and enthusiastic Clinical Pharmacist to join our established and supportive primary care team in Tameside. This is a new practice-employed role, designed specifically around the needs of our patient population and internal service model, embedded within our day-to-day operations and focused on delivering high-quality, patient-centred medicines optimisation services for our registered population. The post has been developed to strengthen our internal clinical capacity, improve prescribing safety and provide additional support for patients with complex and long-term conditions. You will work closely with GPs, nursing staff and the wider practice team as a key clinical role, contributing directly to patient care, service improvement and prescribing quality.

Main duties of the job:

  • Undertake structured medication reviews for patients with complex needs, polypharmacy or high-risk medicines
  • Support patients with long-term conditions to optimise treatment and improve outcomes
  • Identify and resolve medication-related issues, including adherence and safety concerns
  • Deliver patient-centred consultations, supporting shared decision-making
  • Promote patient understanding and self-management
  • Prescribe within scope of competence (Independent Prescriber desirable)
  • Support safe prescribing in line with NICE, MHRA and local guidance
  • Complete medicines reconciliation following hospital discharge
  • Monitor and manage patients on high-risk medications

Job responsibilities:

  • Provide daily medicines-related advice to clinicians and staff
  • Support and improve repeat prescribing systems and workflows
  • Manage medication queries from patients and the practice team
  • Contribute to reducing GP workload through clinical pharmacy support

Quality Improvement & Governance:

  • Support safe, cost-effective prescribing across the practice
  • Participate in clinical audits and quality improvement work
  • Implement safety alerts and prescribing updates
  • Help ensure compliance with CQC standards and medicines governance requirements
  • Work as part of the Awburn House multidisciplinary team
  • Liaise with community pharmacies, hospitals and other healthcare providers to support continuity of care
  • Contribute to clinical meetings and patient case discussions

Education & Development:

  • Provide medicines-related advice and education to patients and staff
  • Support team learning and development in prescribing and medicines use
  • Maintain ongoing professional development and clinical knowledge

Person Specification:

Experience:

  • Registered Pharmacist with the General Pharmaceutical Council GPhC
  • Experience in primary care or a patient-facing clinical role
  • Strong knowledge of medicines optimisation and prescribing
  • Excellent communication and interpersonal skills
  • Experience managing long-term conditions
  • Experience of audit or quality improvement work
