Hours: 35 per week, Mon-Fri, 10am - 6pm
Contract: Permanent
Location: Bristol – Covering Royal Infirmary & Southmead Hospital
Salary: £23,590 - £25,155 per annum
Additional requirement: Applicants must have a full licence (2 years) and access to a vehicle.
Role purpose
As a Service Coordinator, you will be the vital link between hospital referrals and care at home. You’ll manage a team of staff and volunteers to ensure services are delivered safely, efficiently, and with heart. This role supports our mission of connecting human kindness with human crisis.
Responsibilities
- Act as the main point of contact for hospital and community referrals.
- Coordinate and support a team of staff and volunteers to deliver services.
- Monitor service delivery, maintain accurate records, and ensure quality standards are met.
- Attend hospital meetings, manage the service budget, and support frontline delivery when needed.
Requirements
- Experience managing or supervising people and working in a team environment.
- Strong communication skills and the ability to build relationships with health and social care professionals.
- Confidence using IT systems and managing service records.
- A flexible approach, with a willingness to work evenings, be on-call, and respond to emergencies.
