Location: Royal Bournemouth Hospital
Salary: £22,932 per annum, pro rata (£12.60 per hour)
Hours: 28 hours per week (Monday to Friday 10am - 6pm; alternate Weekends and Bank Holidays 9am - 5pm)
Contract: Permanent
Driving: Full UK Manual Driving Licence held for a minimum of 2 years

Responsibilities:- Providing practical and emotional support, including welfare calls, shopping, and home visits.
- Supporting adults after a hospital stay to help them recover safely at home.
- Responding to referrals from clinicians and community health professionals.
- Assessing needs and following a personalised support plan for each service user.
- A kind, calm and confident approach with great people skills.
- Right to work in the UK. We are unable to offer sponsorship for this role.
- The ability to work independently and as part of a team.
- A full UK driving licence and willingness to drive service vehicles.
- Basic IT skills and a commitment to following procedures.
