An exciting opportunity has become available for a caring and dedicated Community Reablement Assistant to join a specialist community reablement service supporting individuals in their own homes across Bournemouth.
This rewarding role focuses on improving lives and building independence by providing short-term, person-centred support to individuals who need assistance following illness, injury, hospital discharge or a change in circumstances.
The aim is to help clients regain confidence, maintain their daily living skills and remain as independent as possible within their own homes and community.
As a Community Reablement Assistant, you will provide practical and emotional support to individuals, helping them achieve their personal goals and agreed outcomes.
You will encourage independence, support daily routines and work alongside other professionals to ensure each person receives the right level of care and support.
