This unique service provides short-term rehabilitation, reablement and end-of-life care, supporting up to 35 clients to regain confidence, improve independence and achieve their personal goals. Located within a welcoming community setting, the centre provides a modern environment where clients receive tailored support from a dedicated multi-disciplinary team, including healthcare and therapy professionals. The service focuses on improving lives and building independence, supporting individuals following illness, injury or hospital discharge, with the aim of helping people maintain their skills, improve wellbeing and remain as independent as possible.
