This is an exciting opportunity for someone who is passionate about person-centred care, community wellbeing and making a real difference to people's lives.As Social Prescriber Team Leader, you will play a key role in proactively identifying and supporting patients, particularly those who are frail, elderly, vulnerable or living with long-term health conditions.
