QOF Care Coordinator – Tyntesfield Medical Group
Tyntesfield Medical Group comprises Backwell Surgery, Long Ashton Surgery, Brockway, and Tower House Medical Centres in Nailsea.
Based at Backwell Medical Centre.
Candidates should be team players with a passion for delivering excellent customer service. The role involves managing our chronic disease register and requires good data awareness and interpersonal skills. A good understanding of Quality Outcome Framework (QOF) targets and a working knowledge of EMIS are desirable. Previous NHS experience, including promoting attendance at health promotion clinics, ensuring timely patient reviews, and conducting basic searches, is preferred.
