The Milton Surgery is a 3 partnered practice with a strong clinical and admin team. We are seeking a motivated and committed Diabetes Practice Nurse to join our supportive practice team. This role will involve running structured diabetes reviews alongside delivering routine treatment room nursing duties. This is an excellent opportunity for a nurse with experience or a strong interest in diabetes care who is keen to develop their skills within general practice.
The successful candidate will:
- Undertake structured diabetes reviews for patients with type 1, Type 2 and prediabetes in line with NICE guidelines
- Provide patient-centred education and lifestyle advice to support self-management
- Support diabetes medication reviews and escalation of care where appropriate
- Deliver treatment room duties including wound care, injections, immunisations, venepuncture and other routine procedures
- Work collaboratively with GPs, nurses and the wider multidisciplinary team
- Maintain accurate clinical records and contribute to QOF and audit requirements
- Commitment to providing high standards of care
