Severnvale Primary Care Network is recruiting an enthusiastic Clinical Pharmacist to join our established pharmacy team. The role involves working 30 hours across 5 days, providing specialist pharmaceutical support to four GP practices, liaising with patients and healthcare colleagues, and delivering high-quality medicines management.
You will lead medicines reviews, optimise prescribing, conduct clinical consultations for patients with long-term conditions, reconcile medicines after hospital.
