Evaluates patients referred for home care services to determine appropriateness for physical therapy services. Establishes a home care plan including an assessment of the appropriateness of the requested services. Assures that the Agency is ready to meet the patients' needs by collaborating with Agency personnel on a regular basis. Explains the Agency's policies and services to the patient and responsible family members during the initial home care assessment. Completes documentation accurately and submits it to the agency office in a timely manner. Establishes and promotes ongoing communication with Agency personnel. Assures customer satisfaction.
