About the Role
As a Team Leader at Greensand House Care Home in Surrey, you will support the home management team in delivering high‑quality care and create a safe, caring, and inclusive environment for residents, visitors, and staff.
Responsibilities
- Lead the person‑centred care planning process, including implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, and developing action plans.
- Assist in the smooth transition of discharges from the home, handling documentation, medication, and keeping residents' next of kin fully informed, where appropriate.
- Monitor the well‑being of each resident, identify changes impacting care and care planning, and ensure changing needs are met and written records are amended.
- Practice safe systems of work across a range of tasks and moving and handling of residents, assessing risks and ensuring personal safety for all.
Qualifications
- NVQ Level 3 or equivalent in care for care homes.
- Two years’ experience in a senior position within a resident group.
- Strong verbal and written communication skills.
- Ability to lead staff and act responsibly in the absence of the manager.
