This role as a Team Leader β Nights at Avery Healthcare offers the opportunity to join a supportive, caring, and award-winning team. You will be responsible for delivering high-quality, person-centered care and supporting the home management team. The role requires strong leadership, communication, and care planning skills to create a safe and inclusive environment for residents, visitors, and staff.
Main duties of the job
As a Team Leader β Nights, your focus will be to support the home management team in delivering high-quality care with a difference. This includes leading in the person-centered care planning process, assisting with resident discharges, monitoring resident well-being, and ensuring safe systems of work. You will demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Job responsibilities
- Leading in the person-centered care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents\β next of kin fully informed, where appropriate.
- Monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
- Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

- Have an NVQ 3 or equivalent in care for care homes.
- Have two years experience working in a senior position with the resident group.

Additional requirements
- A DBS Disclosure check, the cost of which will be met by Avery Healthcare.
- Proof of eligibility to work in the UK.
