Main duties
As a Team Leader at Loxley Park Care Home in Sheffield, you will be responsible for supporting the home management team in delivering high-quality, person-centred care. Your focus will be on creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. You will lead in the care planning process, assist with resident discharges, monitor the well-being of each resident, and ensure safe systems of work across a range of tasks.
About the role
Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Other responsibilities will include:
- Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
- Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
- Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
