Job Description: Resident Advocate-Social Worker

(Full Time)

Location: 119-Southern Utah Veterans Home – Ivins, St. George, UT, 84738 Job Summary:

The Resident Advocate-Social Worker at the Southern Utah Veterans Home will be responsible for providing social work services to residents, including assessment, counseling, and advocacy. The ideal candidate will have a strong background in social work and a passion for working with veterans.

Key Responsibilities

• Conduct assessments of residents\’ social and emotional needs
• Develop individualized care plans in collaboration with residents and their families
• Provide counseling and support to residents dealing with emotional or psychological issues
• Advocate for residents\’ rights and ensure they receive appropriate care and services
• Collaborate with interdisciplinary team members to provide comprehensive care to residents
• Document all interactions and interventions in residents\’ records

Qualifications:

• Bachelor\’s or master\’s degree in social work
• Licensed Social Worker in the state of Utah
• Experience working with older adults, preferably veterans
• Strong communication and interpersonal skills
• Ability to work effectively in a team environment
• Compassionate and empathetic attitude towards residents
