Qualified Social Worker to join our team at AV Care Solutions Limited. This role involves working with vulnerable adults/children and families to assess needs, develop care plans, and ensure high-quality safeguarding support in line with statutory requirements.
The position offers a hybrid working arrangement, allowing a balance between office-based duties, community visits, and remote administrative work.
Key Responsibilities
- Conduct assessments and reviews of service users’ needs
- Develop, implement, and review care and support plans
- Work in line with safeguarding policies and procedures
- Liaise with families, healthcare professionals, and partner agencies
- Maintain accurate and timely case records
- Participate in multidisciplinary meetings and case conferences
- Ensure compliance with social work legislation and council/agency standards
Requirements
- Qualified Social Worker (Degree in Social Work or equivalent)
- Registration with Social Work England (SWE)
- Experience in children’s or adult social care (depending on team)
- Strong assessment, communication, and case management skills
- Ability to work independently and as part of a team
