We are working with an established and growing Home Care provider in Woking, Surrey, who is looking for a caring, organised and motivated individual to join their team as a Care Coordinator. This is a fantastic opportunity for someone who understands care from the front line and is ready to take on more responsibility, develop professionally and build a career beyond hands-on care.
If you’ve had some involvement with staff rotas, scheduling, coordinating visits or arranging cover using ACCESS PEOPLE PLANNER, PASS etc..., that’s a great foundation — and you’ll have the opportunity to build on this with training and support. The company is experiencing an exciting period of growth, with genuine opportunities for career progression and development.
What You’ll Be Doing
- Working alongside the Registered Care Manager, you’ll help coordinate the day-to-day delivery of care, including:
- Assisting with preparing rotas and scheduling, including arranging cover when required
- Helping to ensure the well being of both clients and care staff
- Supporting safeguarding procedures
- Building positive relationships with clients, families and care workers
- Liaising with Social Workers, GP practices, NHS Discharge Teams and other professionals
- Supporting the Registered Care Manager with administration and day-to-day coordination
- Providing personal care in the community when required
- Participating in a weekday & 1-in-4 weekend on-call rota
