The role is varied and extremely rewarding and would therefore suit an experienced Deputy Care Manager or Care Manager who wishes to develop or progress their career within Care Management.
Responsibilities:
- Report to and work closely with the Operations Manager
- Provide the highest standards of Care & Continuity of Care to Clients at all times
- Take enquiries from and provide all relevant information to prospective New Clients and visit them to conduct Assessments
- Liaise with Social Workers, Family & other Professionals to Plan & Manage Care Needs
- Conduct Assessments & Reviews with Clients and produce Client Care Plans
- Ensure the provision of healthcare arrangements
- Use Care planning software to generate staff rotas, payroll and invoices
- Recruitment, induction & supervision of new Care Professionals & Office Staff
- Manage staff performance and conduct for all subordinates to achieve the highest standards
- Liaise with and co-operate with Care Quality Commission inspectors and inspections
- Manage the company’s Audit & Quality Assurance cycle
- Practice maximum integrity in all dealings with Clients' personal and financial affairs
- Provide on call support to clients and carers during evenings and weekends as part of a rota ensuring that all calls are covered & delivering care sessions where required
- Follow local safeguarding guidance and reporting mechanisms to ensure the safety of clients, staff and others
- Promote a positive personal / professional profile within the local community, ensuring the good reputation of the company at all times
