You will provide person-centred support at home, following Care Plans and ensuring dignity and privacy at all times.
You will work with the Clinical Team and help individuals engage with their local community.
The role involves assisting with daily tasks, health and wellbeing, and developing shared interests and activities to improve quality of life.
