About the role
We are looking for a Transition Support Worker who will be based at our school in Nottingham City Centre, Sutherland House School. This is an exciting project to support autistic young people and adults across the East Midlands. We currently have a vacant post focusing on supporting young people and adults transition into adulthood to secure meaningful long-term outcomes. The purpose of the role is to provide a consistent presence in the life of young people aged 14-25 years of age to enable a supportive transition into adulthood. The role will enable long-term positive outcomes, provide signposting for support available including housing and benefits and assisting in employment or further education opportunities for autistic young people and adults including supporting access to work.
What will you do
- Provide one-to-one support to autistic individuals, including both face-to-face and online meetings.
- Be a consistent presence in the life of a young person into adulthood (aged 14-25 years) to support a meaningful and positive transition into adulthood.
- Support young people to identify ambitions for the future.
- Work with internal and external partners to achieve outcomes e.g. accommodation, benefits, employment, education, training, apprenticeships.
- Advocate on behalf of young people internally and externally.
- Liaise with the external evaluation team to design and implement the project to provide a meaningful evaluation report at the end of the two-year project.
- Support autistic young people and adults leaving school with their transition to their next setting.
- Assist in the development and implementation of transition plans for pupils and adults.
- Assist and support in MAP meetings and EHCP meetings to create and develop action plans.
- Work collaboratively with teachers, families, and external agencies to ensure positive transitions for young people into their next setting.
- Provide emotional and practical support to pupils during their transition into adulthood.
- Maintain accurate and up-to-date records including communication logs.
