This position involves working closely with healthcare providers, Service users, and their families to facilitate effective communication and streamline care processes. The ideal candidate will possess strong organisational skills and a compassionate approach to service user\'s care, ensuring that all aspects of the patient\'s journey are managed efficiently. Key Responsibilities Coordinate daily care schedules and allocate care workers effectively Ensure care packages are delivered in line with person-centred care plans Good understanding of CQC regulations and domiciliary care Excellent planning, communication, and interpersonal skills Comfortable using care management software (e.g., Birdie, Access, People Planner) Build strong relationships with clients, families, and external professionals Maintain accurate records and documentation Support compliance with CQC standards and company policies Handle emergency rota changes and staff absences efficiently Supervise, support, and communicate regularly with field staff Rota Management: Develop, manage, and update staff schedules to ensure 24/7 coverage. Promptly address last-minute scheduling changes. Align rotas with clients\' care plans and preferences. Requirements Minimum 1 year of experience in a similar care coordination role. Flexibility to work on weekends is essential. Weekend work will be remote (work from home) as part of your schedule NVQ Level 3 in Health and Social Care (or working towards it) preferred Full UK driving licence and access to your own vehicle is desirable.
