The Role: Care Partner – Support at Home
We are seeking a proactive and compassionate Care Partner to join our home care team. In this role, you will work closely with older people and their families to ensure they receive the right support to maintain their independence, well‑being and quality of life at home.
As a Care Partner, You Will:
- Build meaningful relationships with clients to understand what matters most to them.
- Conduct holistic assessments that identify strengths, needs, risks and personal goals.
- Develop, implement and regularly review personalised care plans focused on wellness and reablement.
- Coordinate and schedule services in partnership with the client, ensuring continuity, quality and value for money.
- Monitor client outcomes and adjust support as needs evolve.
- Work collaboratively with GPs, allied health professionals, community supports and family members.
- Educate and empower clients to manage their own health and safety where possible.
- Advocate on behalf of clients to ensure timely access to the services and equipment they require.
What We’re Looking For:
- Have a current AHPRA registration as a Registered Nurse or Endorsed Enrolled Nurse.
- Possess excellent communication and interpersonal skills.
- Demonstrate a proactive attitude and the ability to take initiative.
- Be committed to continuous improvement and innovation in care delivery.
- Show flexibility and adaptability to meet the diverse needs of our clients.
- Hold a valid driver’s licence and have access to a vehicle.
