We are currently seeking a highly skilled and motivated Band 6 Senior Nurse to join the team in one of the major mental health Trusts in South West England. Working closely with a multidisciplinary team, you will ensure that patients receive timely and effective mental health care, integrating physical and mental health support.

Description:

Mental Health Liaison practitioners utilise clinical expertise, education and management to ensure a seamless service for patients, carers and staff. The priority of the post holder will be undertaking specialist psychosocial assessments, reviews and advice regarding ongoing management of Later Life patients. This may include delivering a range of psychosocial interventions to individual patients and their families as appropriate. The post holder will be assisting and supporting general hospital colleagues in making decisions about treatment and care in complex situations; at all times with a focus on advocating for our patients. The post holder will use high level communication skills, both written and oral and have an ability to engage with challenging and/or acutely distressed individuals in sometimes potentially hostile and/or antagonistic situations. The post holder will provide a communication structure between the general hospital, secondary MH services, primary care and non-statutory services as a means of ensuring the highest possible standards of patient care.

Experience:

  • Knowledge of key areas of liaison psychiatric nursing practice, including the psychological effects of physical illness.
  • Knowledge of complexities of older adult mental health.
  • Skilled at undertaking mental health and risk assessments.
  • 2 years post registration experience of practicing in mental health liaison setting.
