We are hiring Licensed Practical Nurses (LPN) to join our care team at Plymouth Rehabilitation & Health Care Center in Plymouth. Up to a $5,000 sign-on bonus is available! Working with our team and residents will give you purpose in your professional and personal life.

Responsibilities:- Provide direct nursing care under RN supervision to residents.
- Supervise nursing assistants and ensure quality care.
- Maintain current resident care plans.
- Follow physician’s orders and assist with treatments, dressings, and medication administration.
- Document care accurately and thoroughly.
- Visit residents daily to assess needs.
- Report and document any significant findings and contact physicians as needed.
- Maintain safe drug control and emergency procedures.
- Respect facility equipment and supplies, and promote positive communication and relations.
- Adhere to privacy policies and continue professional growth.
- Demonstrate leadership and perform other duties as assigned.
- Graduate of an approved LPN/LVN program.
- CPR certified.
- Current, unencumbered LPN/LVN license in this state.
- Preferably, at least one year of experience in healthcare settings; new graduates are welcome.
