An exciting opportunity has arisen for a Wellbeing Key Worker to join a specialist team delivering personalised wellbeing support across Liverpool. This role focuses on supporting individuals to make sustainable lifestyle changes that improve their health, wellbeing and future prospects. Working with individuals within the criminal justice system, including those on probation or recently released from custody, you will play a crucial role in supporting rehabilitation, reducing reoffending, and helping people progress into employment, education and training.

You will deliver tailored interventions through a mixture of one-to-one sessions, group workshops and digital engagement, ensuring each individual receives the support they need to achieve positive outcomes. The role involves:

  • Managing a caseload of service users, ensuring delivery against agreed targets
  • Delivering personalised wellbeing support based on individual needs and complexity
  • Completing comprehensive assessments, including screening for LDD
  • Developing high-quality, strength-based action plans with service users
  • Delivering support through one-to-one sessions, group workshops and digital platforms
  • Building strong relationships with local services to enable effective signposting
