We are recruiting for a Work and Health Coach to support individuals facing health-related barriers to employment. This is a rewarding opportunity to make a real difference by helping people improve their wellbeing, build confidence and take steps towards sustainable work.

The role focuses on providing holistic, person-centred support, working with individuals experiencing a range of physical and mental health challenges. You will help participants access the right support services across employment, housing, financial wellbeing and debt management, ensuring they receive tailored guidance to move forward positively.

The role involves:

  • Delivering one-to-one coaching to individuals with health-related barriers to employment in hospitals across Manchester and Trafford
  • Supporting participants to improve confidence, resilience and employability skills
  • Creating tailored action plans to help individuals move towards sustainable work
  • Providing advice and guidance on employment pathways, training and job opportunities
  • Signposting and referring individuals to specialist support services including housing, debt management and health services
  • Working collaboratively with external partners and support agencies
  • Monitoring progress and maintaining accurate records of participant activity
  • Meeting performance targets and contributing to programme outcomes

You’ll need:

  • Experience supporting individuals with health-related barriers or complex needs
  • A strong understanding of employability and the challenges faced by disadvantaged groups
  • Knowledge of support services relating to health, housing, debt and financial wellbeing
  • Excellent communication and interpersonal skills
  • A person-centred, empathetic and motivational approach
  • Ability to manage a caseload and work towards targets
  • Strong organisational and administrative skills

Due to the nature of this role, you will be required to complete an online Disclosure and Barring Service (DBS) check.
