Clinical Health Lead – Complex Services (Regional / Hybrid Role)

Salary £44k per Annum plus £3k Car Allowance (Must hold a Professional registration, RGN, RNLD or RMN)

Full-Time Monday-Friday 9-5am. Locations: Cheshire, Liverpool, Stoke, Stafford, Birmingham, Tamworth, & Worcester.

We are seeking a dedicated and compassionate Clinical Health Lead to join our Complex Care team in a regional, hybrid working role. This is a unique opportunity to make a meaningful difference in the lives of adults with complex health needs by supporting the delivery of high-quality, person-centred care within community settings.

About the Role

As Clinical Health Lead, you will provide clinical leadership and specialist guidance to managers and operational teams in relation to individuals with complex health needs. You will play a pivotal role in ensuring care is safe, holistic, and tailored to each person's individual requirements.

Oversight and Support

• Complex health needs across services
• LeDeR reviews and learning implementation
• STOMP/STAMP outcomes and best practice
• Management of the internal clinical/health risk register

Key Responsibilities

• Provide clinical leadership and expert advice to operational teams
• Analyse and review data from the organisation's CRM system to identify trends, risks, and improvement opportunities
• Support medication reviews and promote safe medicines management practices
• Work collaboratively with community nursing teams and wider MDT professionals
• Deliver training, mentoring, and coaching to support workers to build confidence and clinical competence
• Lead on end-of-life or palliative care planning in rare and occasional circumstances
• Contribute as a clinical discipline representative within monthly internal MDT meetings
• Support governance processes, including lessons learned and quality improvement initiatives
• Support in reviewing relevant policies and procedures
• Assessment of new individuals to identify health needs and support required
• Participate in the development of complex health related support plans
• Act as the link with health colleagues for delegated health care tasks
• Ensure any relevant clinical information or queries feed into the annual health review process

Reporting Structure

You will report directly to the Director of Quality & Complex Care and play an integral role within the organisation’s clinical governance and quality assurance framework.

Essential Criteria

Must hold a professional registration, RGN, RNLD or RMN.

Must be a driver with own vehicle, have business insurance and the ability to drive to the above regional locations.
