Ashton Medical Group is looking to recruit an enthusiastic and motivated Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our established same‑day triage hub team. This is an exciting opportunity to work within a forward‑thinking and innovative GP practice delivering same‑day care through a well‑developed multidisciplinary triage model. The successful candidate will play a key role within our central triage hub, assessing and managing patients presenting with acute and urgent primary care needs. The role is ideally suited to someone who enjoys autonomous clinical decision‑making, collaborative working, and working within a fast‑paced primary care environment.
Main duties of the job
- Assessment and management of acute and urgent presentations through telephone triage, online consultation review, and face‑to‑face assessment where appropriate.
Key responsibilities include:
- Clinical assessment and management of same‑day patient presentations
- Telephone and digital triage of patient requests
- Autonomous clinical decision‑making within scope of practice
- Independent prescribing where qualified
- Safety netting and escalation of complex patients where required
- Supporting patient flow and operational efficiency within the triage hub
- Working collaboratively with GPs and the wider multidisciplinary team
- Participation in service development and quality improvement initiatives

Clinical Responsibilities
- Assess, diagnose, and manage acute and urgent presentations within primary care
- Undertake telephone, digital, and face‑to‑face consultations
- Provide autonomous clinical decision‑making within scope of practice
- Prescribe medication independently where qualified
- Request and interpret investigations as appropriate
- Develop and implement management plans for patients
- Identify red flags and escalation of complex or deteriorating patients appropriately
- Provide appropriate safety netting and follow‑up advice
- Maintain accurate and contemporaneous clinical records
- Work in line with local and national clinical guidelines and protocols
Triage Hub Responsibilities
- Support delivery of the same‑day triage model
- Assess incoming patient requests and determine appropriate clinical pathways
- Prioritise patients based on clinical need and urgency
- Work collaboratively with GPs and the multidisciplinary team to ensure effective patient flow
- Contribute to operational efficiency within the triage hub environment
Team Working
- Work collaboratively with GPs, nurses, pharmacists, mental health practitioners, paediatric clinicians, MSK practitioners, care coordinators, and administrative teams
- Support and mentor colleagues where appropriate
- Participate in multidisciplinary meetings and clinical discussions
- Promote a positive and supportive team culture
Quality Improvement and Service Development
- Participate in audit, quality improvement, and service development work
- Contribute ideas to improve patient care and operational effectiveness
- Support implementation of new pathways and innovations within primary care
Professional Responsibilities
- Maintain professional registration and revalidation requirements
- Participate in mandatory training and continuing professional development
- Work within professional codes of conduct and organisational policies
- Maintain confidentiality and information governance standards
- Act in accordance with safeguarding policies and procedures
Essential Criteria
- Registered healthcare professional with relevant professional body registration
- Advanced Clinical Practice qualification or equivalent experience
- Experience managing acute presentations within primary care, urgent care, or equivalent settings
- Excellent clinical assessment and decision‑making skills
- Ability to work autonomously within scope of practice
- Strong communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Good organisational and prioritisation skills
- Commitment to patient‑centred care and continuous improvement
Desirable Criteria
