Leaving Care Support and Integration Officer

Our mission goes beyond individual support; With a proven track record of delivering effective support services, in 2025, we helped house over 1,500 individuals and provided support to over 600 of our service users, helping them to achieve their personal goals and lead fulfilling lives in their new country.
We are now looking for a Support and Integration Officer to join us on a full-time basis for a six month fixed-term contract, working 37.5 hours per week.

As a Leaving Care Support and Integration Officer, you will provide person-centred support, information, advice and guidance to young people who have left the care system and are living in our 18+ supported housing.
Working as part of our Support and Integration Team, you will oversee a caseload, developing and regularly reviewing individual support plans and risk assessments.
You will work closely with Personal Advisers, social workers and other professionals to help young people build their independence and work towards their goals.
Providing practical advice and support, you will ensure they are equipped to navigate housing, finances, education, employment, health and wellbeing.
- Support young people to develop independent living and social skills
- Signpost and refer young people to relevant external agencies, legal services and support networks
- Visit young people in their homes and organise house meetings where required
- Maintain accurate case files and records using our CRM
- Develop relationships with external organisations to support partnership working
- Ensure your work meets safeguarding, confidentiality, equality, diversity and inclusion requirements
Please note, this role will involve travel across Bristol and participation in community outreach programmes.
- To be considered as a Leaving Care Support and Integration Officer, you will need:
- Experience working with young people in a support service setting
- A commitment and desire to support young people and adults to gain life skills and live independently
- Experience of providing advice and guidance, advocacy and culturally sensitive support
- Experience of creating support plans and completing needs and risk assessments
- Experience and knowledge of working with people with complex/multiple support needs
- The ability to effectively assess, manage and continuously review needs and risks
- Strong organisational and planning skills
- An understanding of the importance of safeguarding, in relation to working with vulnerable adults and children
- Confident Microsoft skills - Outlook, Excel, Word and Power Point
