We are now looking for a Leaving Care Officer to join us on a part-time (with the possibility to extend to full-time), permanent basis, working 22.5 hours per week, flexibly across Monday to Friday.

What Youll Be Doing
As a Leaving Care Officer, you will support young people aged 18+ leaving the care system to build the skills, confidence and independence they need for adult life.
Managing a caseload of young people with a range of support needs, you will create and review person-centred support plans and risk assessments. You will provide advice and practical support around housing, finances, education, employment, health, relationships and independent living.
You will work closely with Personal Advisers, social workers and other professionals to strengthen each young persons support network and help them move towards independent accommodation.
Additionally, you will:
- Support young people with budgeting, welfare benefits and accessing care providers - Signpost and refer to external agencies and legal services - Organise house meetings and visit young people in their homes and community - Complete employment, benefits and other relevant forms - Build relationships with external organisations and partner agencies - Carry out community outreach activities across the city
What Were Looking For
To be considered as a Leaving Care Officer, you will need:
- Experience working with young people or vulnerable adults in a support service setting - Experience of providing advice and guidance, advocacy and culturally sensitive support -
Experience of creating support plans, completing needs and risk assessments - Experience and knowledge of working with people with complex/multiple support needs - An understanding of the impact of traumatic experiences on people's wellbeing and Behaviour -
The ability to effectively assess, manage and continuously review needs and risks - An understanding of the importance of safeguarding, in relation to working with vulnerable adults and children
