We are looking to appoint a STAR Trusted Assessor. 18.5 hours per week.
Your Duties Will Include:
- Ability to carry out comprehensive assessments in hospital and community settings across the Borough of Sandwell.
- Identify individual needs and personal outcomes through thorough assessment.
- Develop personalised support plans aimed at meeting identified needs.
- Complete appropriate risk assessments to identify potential risks.
- Implement actions and strategies to minimise any risks identified.
Essential Requirements For The Role Are:
- Minimum of 1 years’ experience working with vulnerable adults and carers.
- Knowledge of assessment frameworks, including risk assessment.
- Ability to travel throughout the borough as required.
- Availability for 7‑day working, including some unsocial hours.
- GCSE English and Maths (or equivalent qualification).
