- Build strong working relationships with dietitians, nutrition nurses, pharmacists, and wider healthcare teams
- Support outpatient pathways and continuity of care between hospital and home
- Deliver services in line with agreed KPIs and service expectations
- Identify, manage, and escalate adverse events in line with policy
- Complete all clinical documentation, activity reporting, and administration accurately and on time
- Quality, safety & governance
- Maintain high standards of documentation and clinical governance
- Complete all mandatory and programme-specific training

- Strong organisational skills with the ability to manage a caseload independently
- A patient-centred approach and interest in long-term condition management
- Confident using digital systems and Microsoft Office applications
- Satisfactory enhanced DBS (or equivalent) and pre-employment medical checks
Desirable:
