We are recruiting for two time-specific projects within the Shared Lives scheme:
- One 12-month project worker
- One 6-month project worker
Both roles involve working 36 hours per week with access to our flex system. The role requires the candidate to drive as part of their duties.
We seek a proactive, experienced, and highly organized Agency Social Worker to join the Shared Lives Scheme on a project-specific basis. Although based within the team, the role functions in a commissioning capacity, aligning local priorities and ensuring strategic coherence across key projects. These projects aim to improve services related to financial arrangements, legal agreements, operational processes, and overall service delivery.
The role involves managing complex tasks such as Care Act reassessments, reviewing Shared Lives placement agreements and licenses, supporting benefit and capacity assessments, and collaborating with finance, commissioning, and governance teams to implement structural changes. The postholder will lead in embedding new frameworks like banding models, fee structures, and policy updates that reflect local needs and broader service goals.
This is a fast-paced, outcomes-driven role requiring strong project management skills, independence, and confidence in balancing operational and strategic responsibilities. The ideal candidate will have a solid background in adult social care, an understanding of welfare benefits and legal frameworks, and a proven track record in delivering service improvements across multi-agency settings.
Main Duties include:
- Assessment & Support Planning: Completing Care Act Assessments and Support Plans, ensuring they are person-centered and aligned with the personalisation agenda. Assessing eligibility for benefits, completing Financial Capacity Assessments, and managing Court of Protection applications. Engaging with service users, families, and carers throughout.
- Care Coordination: Managing Shared Lives placements, ensuring compliance with support plans, conducting reviews, and supporting financial transitions to the Client Finance Team.
- Policy Development: Reviewing and updating placement agreements and licenses, ensuring documentation reflects project developments and financial changes.
- Multi-Agency Collaboration: Working with various teams to develop systems like the Banding Model and Fee Structures, supporting implementation, and preparing documentation.
- Partnership Building: Maintaining relationships with internal and external partners, participating in project meetings, and representing locality needs.
- Stakeholder Engagement: Communicating with service users, families, and carers about changes, gathering feedback, and supporting transitions.
- Quality and Standards: Ensuring compliance with CQC, Shared Lives Plus, and corporate standards, maintaining confidentiality and accurate records.
- Professional Development: Attending training, engaging in supervision, and planning ongoing learning.
- Additional Duties: Ensuring financial adherence and representing the scheme at meetings. Additional responsibilities may be assigned as appropriate.

- DIPSW or equivalent in Social Work
- Social Work England registration
