Details:
The candidate will need to be an experienced social worker with a mental health background.

Core tasks and duties as follows:

Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
Some mental capacity assessments, BI decisions and safeguarding as required
Understanding of Section 117, Attending 117 meetings, ward rounds, professional meetings
Participation in duty rota
LAS experience preferred
Car driver
